Custom Forms
To add the Custom Form Page Part to your page:
- Access the page you are wanting to work with by starting in the main Pages tab and then click on the page name you would like to edit. If the page is not created yet, create it.
- Click the "Add to this Page" link to expand the list of available page parts, and then
click on the 'Form' link. You will notice that the Form Page Part will be added to the page. - Give the form a title and call it what you would like in the 'Form Name' field.
- Next, start adding question types to the form. You can select the following and click 'Add'. (NOTE: Seperate multiple answer questions with comma i.e. 'comma delimited'. So, for example, a drop down with choices of shirt size would be small,medium,large,x-large and this would produce a drop down of choices for shirt size.
- Single Line Text - Good for fields like First Name or Last Name
- Multi Line Text - Good for allowing a comment or message field.
- Drop Down List - Allows the user to see a list of choices but can only choose 1.
- Checkbox List - Allows the user to see a list of choices and choose more than 1.
- Checkbox - Good for a question like "Have Children?" Allows the user to check for Yes or No.
- Radio Button list - Allows user to see a list of choices but can choose only 1.
- Once you have added your question type to the form, give the question a name in the field label column. Continue adding types (questions) until finished.
- Enter a subject line for the email (form results) in the Email Subject field.
- Enter the Email Address(es) that the form results should be sent to on submission of the form. Separate your entries by a comma only and not a comma and a space. The form data will also be stored within the system!
- Enter a confirmation message that you would like to display to the user once the form has been successfully submitted or if you would like to redirect the user to a page on your site on successful form submission, choose the 'Redirect to another site page' option and select the page.
- Click Save.
To enable or disable the SPAM control feature, check or uncheck the box for 'Disable Form SPAM Blocker'. If enabled a simple math question will be displayed forcing someone to answer. This prevents spam entries.
Viewing and Downloading Results
To view the results of this form submission, access the page you areworking with, find the Custom Form page part in question, and click the'View Results' button. In the pop up you will see up to the minute results along with an option to export the data if you need to do so. When choosing to export the data you can choose 1 of 2 date export options.
1.) Export range: No range - All Data - will give you all data since the form was created.
2.) Export range: Within a range - Allows you to chose a beginning and ending date to pull your results from.
To edit the Custom Form, you can simply change the values within the Custom Form page part and click save.
To delete the Custom Form page part completely, click the 'Red X' by the Custom Form name, and confirm the deletion.
